I am a 26 year old male and have lost about 20 pounds in the past six months (am now at about 175). My fasting BG was 340 mg/dl, A1C was 10.9%, and high ketones in my urine. I was told by a walk-in clinic internist I was type 2 and started on Metformin (1000 mg twice a day), only ate greens, egg whites, and turkey, and exercised for about 15-20 minutes after each meal. My fasting BG was down to about 95 and postprandial under 150 after 4 days. I saw a new internist who confirmed the type 2 diagnosis and treatment, but gave no advice on diet, except for fat intake.

Five days after the initial diagnosis and start of treatment, I was able to see an endocrinologist who told me I was type 1 due to lack of family history (with the exception of a grandmother who developed diabetes in her sixties after becoming very overweight), age, body type, and ketone levels.

As far as I understand, there is no definitive test to determine if someone is type 1 or 2. The type 1 antibodies are not necessarily present in all type 1 patients, and the c-peptide test is not always reliable.

Additionally, the endocrinologist I saw prescribed 16 units per day of Lantus and a sliding scale of Novolog before each meal starting at 5 units, and going much higher depending on my preprandial BG. I have decreased the Lantus dosage to 14 units due to low fasting BG levels and am now eating 2-4 servings of carbohydrate per day. I have yet to take one unit of Novolog since my fasting and postprandial BG levels have been in the 70-105 and 95-130 range, respectively. Given the ?500 Rule? (500/16), I should only need 1 unit to cover 31 G of carbohydrate assuming I produce no insulin at all, which the endocrinologist did not believe to be the case. Would that 5+ units have put me into a dangerously severe hypoglycemic state? Should I look for a new doctor, or are my fears unfounded?

Let me expand a bit on the weight issue. I had lost the 20 lbs before being diagnosed, which is one of the factors the endocrinologist used in his type 1 diagnosis. I am 6?1?, meaning I was probably 5-10 pounds overweight six months ago. I have gained (or stopped losing weight at least) since I began treatment.

I suppose my real question is whether I should be going to see a different endocrinologist to re-evaluate both the diagnosis (1, 2, or LADL all seem like possibilities) and treatment (I honestly think if I had taken the 5+ units of Novolog, it would?ve sent me to the hospital unless I had eaten something equivalent to a loaf of bread).

You can test for type 1 vs type 2 by checking your endogenous insulin levels. That is, if you are producing insulin as in type 2, you can measure it. Type 1 you have no insulin. Looks like you were doing well with metformin, so I don?t understand why the endocrinologist was convinced you had type 1. Getting on insulin has its risks! not only can insulin cause hypoglycemic affects that can kill you, but too much insulin causes atherosclerosis and heart disease.

Age is not a reason to say you are type1, since many kids now are diagnosed with type 2. Bottom line is, you need to find out if you have endogenous (from/in your body) insulin production. You can test for antibodies too.

Researchers from the National University of Singapore and National Cancer Centre Singapore found that Asian breast cancer patients turned to mahjong, qi gong and complementary alternative medicine to cope with cognitive changes

A recent study by National University of Singapore (NUS) researchers revealed that Asian breast cancer patients who had received or were undergoing chemotherapy treatment showed symptoms of "chemobrain", in which they encounter memory loss, difficulty in decision making and speech problems.

This first-ever qualitative study conducted among Asians, led by Associate Professor Alexandre Chan and spearheaded by PhD candidate Ms Cheung Yin Ting, from the Department of Pharmacy at the NUS Faculty of Science, was conducted on 43 breast cancer patients who had completed, or are receiving chemotherapy treatment.

The team, comprising researchers from the Department of Pharmacy at NUS and the National Cancer Centre Singapore, conducted the focus groups last August.

The findings revealed that patients are not aware of the potential cognitive disturbances that may result after cancer treatment. They attributed the lapse in their cognitive functions to fatigue, anxiety and mood changes. The patients who participated in the study expressed that they were overwhelmed by the physical side-effects of chemotherapy that they were oblivious to the cognitive changes.

Among the participants who are married, many had expressed frustration as cognitive deterioration had limited their roles as homemakers. Much of the negative impact on the family originated from their own expectations of themselves, as they were unable to fulfill their duties as mothers and wives in the family due to the cognitive deficits.

The findings are consistent with similar studies conducted on Caucasian patients.

Coping with cognitive changes

Despite the awareness that their cognitive functions could potentially be affected, a majority of the participants are still receptive to chemotherapy and are amenable to coping strategies of their cognitive changes.

To cope with their cognitive changes and to prevent deterioration, the participants turned to mind stimulation activities such as mahjong and practised qi gong. Some participants also consume complementary alternative medicine such as walnut and ginkgo extracts to regulate their moods and to reduce the severity of their fatigue.

Commenting on the findings, Assoc Prof Chan said, "Our study showed that post-chemotherapy cognitive changes have significantly impacted the family and working lives of Asian cancer patients. As there is evidence that cognitive disturbances can differ among different Asian ethnic groups, well-designed epidemiological studies are needed to quantify the prevalence, severity and impact of this problem in Asia."

"A culturally relevant approach should be adopted to evaluate and manage cognitive changes in these patients," he added.

The findings of this study were published in the online version of Annals of Oncology on 6 March 2012.

To address the impact of "chemobrain" among breast cancer survivors, Assoc Prof Chan and his team are currently conducting a prospective, large-scale, longitudinal study to evaluate the degree of cognitive changes experienced by local cancer patients.

S.Africa's Aveng H1 earnings slide by 30 pct

JOHANNESBURG (Reuters) - South Africa's biggest construction group Aveng reported a 31 decline in first-half profit, hit by contract execution problems in Australia and an industry-wide slump at home.

Aveng, which also operates in the Middle East, said diluted headline earnings per share totalled 67.5 cents in the six months to end-December compared with 98.2 a year earlier.

Headline EPS, the main profit measure in South Africa, strips out certain one-time items.

South African construction firms avoided the 2008 global building slump thanks to the construction boom in the run-up to the 2010 soccer World Cup.

But they are now struggling to replenish their order books as the South Africa government delays rolling out its nearly 1 trillion rand infrastructure investment package.

Aveng said its two-year order book rose by 24 percent to 46 billion rand in the past six months, underpinned by strong demand from mining and energy sectors in Australia.

Shares in the company have gained about 11 percent so far this year, outpacing a 5 percent gain the JSE All-share index.

If you think about it, putting a parade together must be a terribly complicated process: Getting all those floats, arriving at different times from different directions, to their assigned positions in a queue?

Equally difficult might be asking thousands of parade-goers from all over New England to carefully choose between the trash can and the recycling bin. But that is exactly what the Newport Energy and Environment Commission is asking of visitors to the 56th Annual St. Patrick's Day Parade.

This year, the commission is partnering with Newport's Clean Cities program, the Rogers High School Green Team, and Rhode Island Resource Recovery Corporation to recycle all the cans and bottles along the parade route.

The benefits of recycling these materials are three-fold: First, recycling materials like aluminum reduces the amount of energy and raw materials needed to make new cans. Second, it increases the lifespan of the Rhode Island Central Landfill, which is scheduled to close in 20 years or so, depending on how much we reduce the amount of waste we send there. And third, our city receives a substantial rebate from RIRRC for the amount that we recycle as opposed to what we landfill. In short, it is a win/win/win proposition, for both the environment and for our city coffers.

So when you come to Newport to view the amazing and wonderful parade, note that there will be recycling containers next to the trash containers, and please participate, keeping in mind that high school students and other volunteers will be hand sorting to divert recyclables and earn dollars for our city. In short, be doubly green this St. Patty's Day by recycling, and please don't make us sift aluminum cans out of half-eaten portions of bangers and mash!

Gay couples make wedding plans amid angry Catholic sermons

"Let me not to the marriage of true minds/ Admit impediments," declared the bard of Stratford in his 116th sonnet. And at the Globe theatre in central London on Sunday ? even as Catholics were being urged from thousands of pulpits across the country to oppose gay marriage ? there was no shortage of same-sex couples ready to heed his encouragement.

At the Designer Civil Partnership show at Shakespeare's erstwhile theatre, excited couples discussed the colour scheme of invitations, whether wedding "favours" were a necessary part of the big day ? and the decision of the Catholic church to wage war against government plans for gay marriage.

"I think it's disgusting. We are not second-class citizens and the idea that this archaic institution should dictate how we live our lives is appalling," said Matt Turrell, 37, a photographer specialising in civil partnerships. "At the end of the day, the union of two people should be about love. Why should we be denied the right to express that publicly?"

On Sunday a letter from two senior Catholic archbishops was read in 2,500 parish churches during mass, arguing that a change to the law would reduce the significance of marriage. Archbishop Vincent Nichols, the leader of the Catholic Church in England and Wales, and Archbishop Peter Smith, the archbishop of Southwark, urged their flock to sign a petition against the move, telling them it was their "duty to do all we can to ensure that the true meaning of marriage is not lost for future generations".

This has a profound impact on people like Evelyn Len, 29, a practising Anglo-Catholic who is due to marry her partner next May. They will have a civil ceremony, but have found a priest who is willing to do a religious blessing. "I would like to get married in a church, and it's very frustrating because I think lots of priests would like to be able to," she said. She was "at peace" with both her religion and sexuality, she added. "I go by what I think Christ would say. I try and keep that in mind when people get angry about it ? people have taken these stances, not God."

Her partner, Shelley Webster, 29, saw the debate in a positive light. "When I was a teenager, I never thought the stage when I would be able to have a civil partnership would come," she said. "We've got a long way to go, and the pace of change is slow, but at least it is happening, it is being discussed."

With a string quartet playing in the entrance hall, intense discussions on whether ushers should wear matching cufflinks, and stalls displaying everything from chocolate macaroons to crystal-encrusted table centre pieces, this was a wedding fair much like any other.

But gay couples are still made to feel excluded because they cannot marry in the same way as heterosexual couples, according to Chris Ford, 30, and his fiance Andrew Ogilvie, 32. The couple, both nurses, were told they could have no religious element to their service and described it as the first barrier they had faced as gay men.

"I was gobsmacked," said Ogilvie. "Automatically you feel second class, that your union is not valued in the same way. It's not like we are all going to be marching into Catholic churches in bridal dresses, but you just want to have the option. Civil partnerships are good, but they are not perfect."

The archbishops' decision to label the changing of the legal definition of marriage a "profoundly radical step", which would strip the union of its "distinctive nature", was hurtful to religious gay people, said Shaz Riley, 46, director of the Butch Clothing Company, who was giving advice to potential brides who wanted to avoid a big white dress.

"I have gay Catholic friends who will be no less than heartbroken at what is happening today," she said. Reacting to the archbishop's comments that "marriage is intended for the procreation and education of children" ? and that this should exclude gay people ? she said: "It's a very sad day. That the church can tell us we will go to hell, or that we can't have families in this day and age, is astonishing."

She and her partner Sue ? who introduces herself as Mrs Sue Riley ? had a civil partnership ceremony, but would have preferred to have married. "I refer to her as my wife, she is my wife and we are very happily married, no matter what the law might say."

While the new dean of St Paul's Cathedral the Very Rev Dr David Ison has called on the Church of England to embrace gay marriage, the archbishop of York, John Sentamu, joined the Catholic-led opposition. "I happen to believe that to change the law in the end would be forcing an unjustified change," he told the BBC's Andrew Marr Show. But the prime minister has made his support for gay marriage clear. "We believe people should have the option of civil marriage, irrespective of sexual orientation," a spokesman said this week.

Gino Meriano, 49, founder of the Designer Civil Partnership show, cringes at the term "civil marriage". Doling out fizz to anyone within striking distance ("Clare will see you right for Lambrini"), he is in the unusual position of campaigning for civil partnerships for straight people. "This is the 21st century: gays want marriage, straights want civil partnerships, there are single mothers, there are househusbands. You can't stop the progress of society," he said. "We are not hard done by, we have no chip on our shoulder ? we just want equality."

Standing in the sunshine overlooking the Thames from a balcony at the Globe, Natasha Marshall, 31, and Debbie Cross, 38, tuck into the bubbles, chatting about the wedding rings they have just chosen for their civil partnership in September. The pair, who have been together for 13 years, would have liked the option of a civil wedding, but seem unconcerned about the fact that they will not yet be able to have a religious ceremony. "Church weddings are boring anyway," said Cross. "We're going to have a lot more fun than that."

Military notebook: Raptor tearing into air show

It?s official. The world?s most advanced fighter jet will make an appearance at the upcoming air show at Robins Air Force Base.

Organizers previously said they were hoping to get an F-22 Raptor at the show, but last week it was confirmed. The show is scheduled for April 28-29.

The Raptor is the Air Force?s newest aircraft, with advanced avionics and radar-evading capability.

Most people know Robins is a 24/7 operation, but have you ever wondered what really goes on there after dark?

The base Public Affairs staff put in some late-night hours recently in an effort to show people unsung heroes of the graveyard shift. Photographers spent time on various nights with different units, photographing various work going on when most base workers are at home asleep.

No iPads for Special Ops

Air Force Special Operations Command had planned to buy 2,861 iPads for flight crews.

The move was intended to replace thousands of pages of paper flight instructions, according to a story in the Air Force Times. Civilian pilots had already turned to iPads to replace binders that could weigh as much as 40 pounds. The purchase, however, was cancelled but the reason was in dispute.

Some said it was because of security concerns, but a command spokesman denied that, saying it was cancelled to ?explore options.?

Air Mobility Command, meanwhile, plans to go forward with a move to buy 18,000 iPads or an equivalent model, the story stated. The command said only non-classified information would be on the computers.

The Student Veterans Association of Macon State College is having a dinner and silent auction on April 3 in the banquet hall of the Professional Sciences and Conference Center on the Macon Campus.

Col. Evan M. Miller, 402nd Maintenance Wing commander at Robins, is the featured speaker. The auction begins at 5 p.m., with dinner following at 7 p.m. The cost is $35 per person, with proceeds going to support a scholarship for veterans. The auction and dinner are open to the general public.

Donations to the auction are being sought, and items may include anything from artwork, jewelry and trips to gift baskets and event tickets.

(AP) ? Suspended by the NCAA right before last year's Big 12 tournament, Perry Jones III seemed in a mood to make up for lost time.

So the 6-foot-11 Baylor sophomore made all eight of his shots in the first half Thursday and wound up with a career-best 31 points while leading the hot-shooting Bears to an 82-74 victory over Kansas State. Jones was 11 for 14 for the game.

"I guess I was just in the zone today," said Jones, who also had 11 rebounds for his eighth double-double in a season that's been a bit of a disappointment. "Just being aggressive and more assertive."

Shortly before the Bears were to board the plane for Kansas City last March, the NCAA suspended Jones because of a mortgage loan his mother had accepted two years earlier. The school vigorously contested the ruling, but it stood, and he eventually had to sit out the first five games of this season as well.

But all that was erased in the finest game of his career. The fourth-seeded Bears (26-6) took charge in the first half when Jones scored 11 straight points. He was 2 for 2 behind the 3-point arc, put down dunks and tap-ins and follow shots.

"It's pleasing because I got my teammates to give me the ball in the right position, my teammates encouraging me to shoot shots I don't normally take," he said. "And they told me to do it like I did it in practice."

The Bears did not let Jones forget what happened last year. When he boarded the plane to come to KC this week, they all gave him a standing ovation.

"My teammates reminded me when I got on the plane," he said with a grin. "But I mean, other than that, it was all laughing and jokes. It wasn't nothing serious."

It wasn't all laughing for the Wildcats (21-10), who had won four of five and appeared to be peaking just in time for the postseason, as they did a year ago.

"We were so bad defensively today," said coach Frank Martin. "Baylor was good. They're a heck of a basketball team. I don't want to take away from them, their moment, because they won the game. But defensively, we were so selfish today. We didn't help each other."

Brady Heslip had 15 points and Pierre Jackson added 13 points and eight assists for Baylor, which shot a sizzling 57 percent while debuting bright, fluorescent yellow uniforms.

"Coaches always want to win and do what you need to do to win," said Baylor coach Scott Drew. "But Perry's one guy I think our whole staff and team really felt good that had a great game."

Even Martin was happy to see Perry get his vindication.

"I told Perry after the game, 'Man, keep being who you are,'" Martin said. "Don't give in to perception and public nonsense. Be who you are, man. He's a beautiful kid and he represents class and he's a very talented player."

Jordan Henriquez, often left unattended in the middle of Baylor's zone defense, scored a career-high 22 points for Kansas State, which had split with Baylor in the regular season.

Martavious Irving, in the meantime, was on a seven-point run for the Wildcats. Irving's 3-pointer sliced Baylor's six-point lead in half, then Jones, with a nifty feed from Jackson, connected for Baylor.

Henriquez's three-point play pulled Kansas State even at 27-all, then Heslip drove in for a layup, drew a foul and sank the free throw and followed that with a 3-point bucket.

Jackson's left-handed floater in the final seconds sent the Bears into halftime with their biggest lead, 45-36.

McGruder had 14 points for Kansas State and Angel Rodriguez had 12.

Li wins to set up all-China contest at Indian Wells

Reigning French Open champ Li Na of China rallied to win her opening second-round match at Indian Wells, beating Galina Voskoboeva 3-6, 6-3, 6-3.

Li, who is the first Asian player to win a Grand Slam title, had two aces and won 67 percent of her first serve points in the two hour, six minute match.

The eighth seeded Li advances to the third round where she will face countrywoman Zheng Jie, who beat Michaella Krajicek 6-1, 6-3 earlier on Friday.

In Ohio, second-place Santorum shows staying power

STEUBENVILLE, Ohio/WASHINGTON (Reuters) - With victories in the Bible Belt and the Great Plains on "Super Tuesday," Rick Santorum continues to bedevil Mitt Romney.

Santorum's ability to attract blue-collar and evangelical conservatives was evident again on Tuesday - and led the former senator from Pennsylvania to within a whisker of Romney in Ohio, the most coveted prize among 10 state contests for Republican presidential candidates.

Santorum and Romney, a former Massachusetts governor, are battling for the Republican nomination to challenge Democratic President Barack Obama in the general election on November 6.

Santorum secured victories in Tennessee, Oklahoma and North Dakota on Tuesday, while Romney notched wins in Virginia, Vermont, Massachusetts, Idaho and the key state of Ohio.

In the final days before Super Tuesday, Santorum said the contest against Romney was a battle of David versus Goliath. In the spending contest, that claim was true: Santorum and his allies were outspent by Romney and his supporters by more than 4 to 1 in Ohio.

At a campaign rally in Steubenville on Tuesday evening, Santorum struck a populist tone, describing his candidacy as devoted to Americans who have fallen on hard times.

"This campaign is about the towns that have been left behind and the families who made those towns the greatest towns across this county," Santorum told the crowd gathered in a high school gymnasium not far from his Pennsylvania home.

Those hit hardest by the down economy appeared to have sided with Santorum over Romney in Ohio.

In CNN's exit polls, Ohio voters without college degrees and earning less than $100,000 voted for Santorum over Romney. Voters earning more than $100,000 and college-educated voters cast more ballots for Romney than Santorum.

On Tuesday evening, Santorum forecasted stormy days ahead for Americans should Obama remain in office.

"This is the beginning of the end of freedom in America," Santorum said.

Since defeating Santorum in the neighboring Rust Belt state of Michigan, Romney, a former private equity executive, has pressed the notion that Santorum is unqualified to be president on economic issues.

"ECONOMIC LIGHTWEIGHT"

"Rick Santorum is a nice guy but he is an economic lightweight," Romney said during a town hall in suburban Columbus last week.

That argument resonated in Ohio, where more than half of voters said the economy was the most important issue in the campaign. Forty-one percent of those voters supported Romney. Thirty-three percent of voters focused on the economy said they support Santorum.

A major test for Romney, a one-time leader in the Mormon Church, has been his ability to appeal to voters who identify themselves as evangelical or born-again Christian. Santorum, a devout Catholic who has made his faith and religious freedom central to his campaign, continues to win over those supporters.

Santorum's appeal to his religion has led the candidate to court controversy. His statement that the views of President John F. Kennedy, a Catholic, on the role of religion in the public sphere made him want to "throw up" cost him support among fellow Catholics.

Still voters choosing their candidate on social issues are lining up behind Santorum.

Ohio voters who said that abortion was the most important issue in the campaign voted for Santorum over Romney 3 to 1, according to CNN's exit polls.

Thirty-one percent of Ohio voters identified themselves as very conservative. Forty-eight percent of those voters said they cast their ballots for Santorum, while 29 percent of those voters picked Romney.

In Ohio, where Santorum topped Romney 47 percent to 31 percent among self-identified evangelicals in exit polls, voters said they backed Santorum because he shared their views.

"Santorum's values are more like mine - more conservative," said Katherine Frencz, 36, who is from Hilliard, a suburb of Columbus, the state capital. "I see Romney as more liberal and not sincere in his beliefs. He doesn't really know what he stands for."

"I want someone like Santorum in the White House who has the moral clarity to make the right decision," said Lonnie Vestal, 36, a conservative pastor and teacher from Mason, Ohio.

Santorum will visit Kansas on Wednesday before heading south to Mississippi and Alabama which hold primaries next week.

The smartest guy on Wall Street may be Watson

IBM has been seeking out practical applications for Watson's abilities in banking, retail and health care, among others.

Wall Street to Watson: Come here. I need you.

The IBM computer that defeated a slew of smart people on the game show "Jeopardy!," thus proving that a computer can answer questions posed?in everyday language in a way that humans can, has been hired by?Citigroup to help?improve banking, Citigroup?announced Monday.?

"Citi will evaluate ways that IBM Watson technologies can help analyze customer needs and process vast amounts of up-to-the-minute financial, economic, product and client data," Citigroup said in a statement.

In the year since Watson beat its human rivals on the popular game show, IBM has been seeking out practical applications for Watson's abilities in banking, retail and health care, among others. The company recently formed an advisory board to explore ways that Watson can be used to improve health care. Last year, IBM announced that Watson would be working with Wellpoint Inc. to improve patient care.

Now, maybe Watson can figure out how the get Washington to work better.

In a landmark ruling from the United Kingdom?s High Court, legal claims can now be served via Facebook. Although this has happened before on rare occasions, typically when a defendant cannot be easily identified or located offline, this opens the door for the practice to become much more common. As more and more people construct their primary identities online and leave little in the way of an official paper trail, the courts have been forced to adapt.

While some question the efficacy of serving claims via the internet, it is clear that the trend towards greater cooperation between the worlds of traditional legal practice and modern online living will continue. However, reaction towards the ruling has been mostly negative, or so the comments left on the original article as well as opinion pieces suggest. Some commentators are scoffing at the notion that the courts can assume any connection between an online persona and the real world. One remarked that his Facebook account was actually in his horse?s name, and another suggested that would be criminals avoid charges through use of an alias. Yet others see this ruling as an example of ?power-hungry? judges ignoring due process and diverting the course of justice in order to serve the interests of the powerful.

But what are the experts saying? Lawyers and tech experts also appear to be skeptical of how widespread the impact of the ruling will be, but they do admit that using Facebook is a novel way of tracking down hard to reach defendants, and can be useful when desperation sets in. It is also worth noting that the practice is routine in Australia and New Zealand.
